narcissism is a defense against borderline personality disorder. I could easily believe that it could also be a defense against bipolar disorder. I’m not a psychologist but I had a college buddy who was bipolar — he once told me he was diagnosed as such and on medication — and over the years I saw him drift into obvious narcissistic behavior. He started drinking heavily, and I suspect he was off his medication. He became so offensive that I eventually cut off contact.
